Transition Assessment Record
At St.Francis we complete a Transition Record for children who are making the transition to nursery or school. The purpose of this assessment record is to inform the new practitioner of what the child can do, what the child likes, dislikes and gives an informed view of their needs. The record is completed by the child's keyworker and is based on observations and keyworker records. Parents are given the opportunity to record their views and opinions and the child's views are also recorded. A copy is given to the parents, the new nursery/school and one is kept on file at the pre-school.
The assessment record is a summary of the child's development and learning and is based on observations throughout the Foundation Stage. This includes:
- Attitudes to learning
- Personal, social and emotional development
- Communication, language and literacy development
- Problem solving, reasoning and numeracy
- Knowledge and understanding of the world
- Physical development
- Creative development
Parents and children are always given the opportunity to visit the new school or nursery prior to them starting and at St. Francis we would welcome visits from either the Nursery or School to help make the transition process easier for the child. If a need arises staff at St. Francis will visit the nursery or School with the child and parent if it is felt it will benefit the child.
